RECOMMENDATION:
Staff recommends that the City Council authorize the City Manager to:
execute an Agreement with the Beach Cities Health District (BCHD) to provide care management services in the amount of $186,575 over a five-year period.

FISCAL IMPLICATIONS:
The agreement allows the Beach Cities Health District to invoice the City of Manhattan Beach quarterly for costs and expenses related to the provision of care management services in an amount not to exceed $186,575 over a five-year period. The annual cost ($37,315) is budgeted in FY 2017-18.

BACKGROUND:
This agreement is not a new service delivery model; it has been in place since 2001. Beach Cities Health District is the largest preventive health agency in the nation. It has served the communities of Hermosa Beach, Manhattan Beach, and Redondo Beach since 1955. The organization offers an extensive range of dynamic health and wellness programs, with innovative services and facilities to promote health and prevent diseases in every lifespan-from pre-natal and children to families and older adults. The Beach Cities Health District provides care management services to older adults and people with disabilities in Manhattan Beach. The care management services include an in-home assessment, consisting of an evaluation of needed services, major medical conditions, medications, social support, financial resources, home safety and emotional and mental health.
