RECOMMENDATION:

Staff recommends that the City Council authorize the City Manager to:
execute an Agreement with the Beach Cities Health District (BCHD) to provide care management services in the amount of $186,575 over a five-year period.

FISCAL IMPLICATIONS:

The agreement allows the Beach Cities Health District to invoice the City of Manhattan Beach quarterly for costs and expenses related to the provision of care management services in an amount not to exceed $186,575 over a five-year period. The annual cost ($37,315) is budgeted in FY 2017-18.

 

BACKGROUND:

This agreement is not a new service delivery model; it has been in place since 2001. Beach Cities Health District is the largest preventive health agency in the nation. It has served the communities of Hermosa Beach, Manhattan Beach, and Redondo Beach since 1955. The organization offers an extensive range of dynamic health and wellness programs, with innovative services and facilities to promote health and prevent diseases in every lifespan-from pre-natal and children to families and older adults. The Beach Cities Health District provides care management services to older adults and people with disabilities in Manhattan Beach. The care management services include an in-home assessment, consisting of an evaluation of needed services, major medical conditions, medications, social support, financial resources, home safety and emotional and mental health. In some instances, this may include screening for cognitive ability and depression. Once the assessment is complete, the Care Manager and the resident discuss any needs and identify solutions.   In July of 2001, the City of Manhattan Beach entered into its first five-year agreement with Beach Cities Health District agreeing to pay a portion of the costs associated with “Senior Care Services.”  The agreement was renewed in 2006 and 2011.  The new agreement will provide services through 2021.  Although a contract is currently not in place, Beach Cities continues to provide senior care services. This agreement designates one care manager for all City of Manhattan Beach eligible residents to provide the needed services.

 

DISCUSSION:

The City's Older Adult Program works closely with Beach Cities Health District to provide programs and services to Manhattan Beach older adults.  By the City supplementing the existing care management service, Beach Cities provides a dedicated care manager, who is available to senior and disabled residents and their families, city staff, police and fire to provide consultation, assessments, information and referrals, access to subsidized services and volunteer programs and on-going support. Additionally, Beach Cities is able to provide weekly outreach to the seniors by attending programs at Joslyn Center. This outreach enables the care manager to establish relationships with the seniors so they see him or her as a trusted resource for their questions or concerns. The care manager also meets with City Older Adult Program staff regularly to discuss referrals and concerns.
